Eligibility
Choice Care at Home serves people widely varied in age and lifestyle. This assistance can be provided to persons who are elderly or have physical, intellectual, or developmental disabilities. Choice Care at Home serves both children and adults in need of daily living assistance.
You can be served if you meet the medical criteria for this Program, currently hold a West Virginia medical card (Medicaid), or are Medicaid eligible and are age two or above. Your medical condition will be assessed when Choice Care at Home has been notified of your interest in Personal Care. Medical need is based on your ability to care for yourself, not any particular diagnosis. And remember, you only need to call us. We will travel to you to conduct any needed paperwork and assessments.
Who Delivers this Service?
Registered Nurses and Personal Care Aides provide your Personal Care Services. These trained professionals have the experience and skills necessary to give you peace of mind as you or your loved ones are cared for at home. The Registered Nurse will conduct assessments, write plans for your care, and monitor the delivery of direct care services in your home. The Nurse will also serve as a resource person for you and your family. Choice Care at Home strives to ensure that you and your family actively participate in planning services. This allows you to get the services you need when you most need them. The Personal Care Aide will help you with the actual day-to-day tasks needed to keep you independent, such as cooking, bathing, grooming, and housekeeping.
