Aged and Disabled Waiver (ADW) Personal Attendant Services

Choice Care at Home adheres to the philosophy that all persons have an inherent right to live as independently as possible and strives to empower individuals to remain in their homes and communities rather than enter a nursing home. The Aged and Disabled Waiver (ADW) program is an alternative to nursing home placement that allows a member to remain at or return to their home. The program provides person-centered care that promotes choice, self-reliance, respect, and community integration. Members requiring personal attendant services must apply for the Aged and Disabled Waiver program. The application for ADW services is called a Medical Necessity Evaluation Request (MNER). We “hold your hand” throughout the application process to begin care. We send the form to your primary care physician for completion. Choice Care at Home then sends Acentra the completed form for review, and Acentra schedules an assessment. If approved for care, members will receive a selection form to indicate which homemaker agency they desire to provide the personal attendant services and which agency they wish to provide their case management services. Due to Conflict-Free Case Management, members may only have the same personal attendant and case management agency if the Bureau of Senior Services (BOSS) grants an exception.

Applicants must be at least 18 years of age and meet financial and medical eligibility requirements to receive personal attendant and case management services. To qualify for services, an individual must have five deficits in areas of daily living, including bathing, dressing, eating, grooming, restroom assistance, moving around home or community, or other areas of need. Once someone is accepted, a program slot must be available for participation.
