Veteran's Services
Choice Care at Home provides in-home services to our nation’s veterans to allow them to remain independent in their homes. Serving those who have served is an honor for the company and allows us to provide veterans with an alternative to nursing home placement. The company works collaboratively with Veteran’s Administration Medical Centers across the state to coordinate care for veterans. It is an honor and a privilege to serve our veterans. Veterans must have established care with the local VA Hospital to be referred by the hospital social worker to receive care from Choice Care at Home and an assessment completed by our licensed Registered Nurse.
